For the extract be sure to get pure vanilla extract, not imitation.
How to Make Caramel Macchiato Creamer
- In a small saucepan over medium heat, combine sugar and water. Bring to a boil then reduce to a simmer. Stir until the sugar is fully dissolved.
- Whisk the caramel sauce into the simple syrup mixture over medium-low, stirring constantly. Do this for about 2 minutes.
- Remove the caramel syrup from heat and stir in vanilla. Set aside.
- Add heavy cream and milk to a jar 16 ounces or larger.
- Pour the caramel macchiato syrup into the cream mixture and shake to combine. If it’s easier, you can combine the cream mixture and caramel syrup in the saucepan, then transfer to bottle.
- Store the caramel macchiato creamer in the refrigerator for 7-10 days or less if either the heavy cream or milk has an earlier expiration date.

Use espresso.
Espresso is bold and strong and stands up to all the milk and cream in a caramel macchiato. Regular coffee just won’t cut it.
Steam the creamer.
This homemade caramel macchiato creamer is ideal for steaming since it’s made with real cream and milk (not heavy sweetened condensed milk).
Steam the creamer to about 160 degrees using the steam wand on your espresso machine. This temp is literally the sweet spot, bringing out the natural sweetness in the milk.
Otherwise, steam the creamer in an electric milk frother. You want one with a max capacity big enough, like the Breville Cafe Milk Frother, to heat up enough milk to make a latte or macchiato.
Layer the macchiato.
- Add the steamed creamer and foam to a 12 ounce mug or larger. (I steam 6 ounces of creamer.)
- Pour 2 shots of espresso (about 4 ounces) over the creamer.
